Design of the Hospital at Home healthcare delivery and organizational model.


Developing the organizational and clinical model for a new Hospital at Home service — expanding access to quality care beyond hospital walls across the Greek healthcare system.
The Challenge
Hospital at Home services offer a clinically proven model for delivering acute and post-acute care in patients' homes, reducing unnecessary hospitalizations and improving patient experience. Greece had yet to establish a structured, nationally coordinated service of this kind, creating a significant gap in the continuum of care.
Our Approach
We supported ODIPY in designing and implementing Greece's Hospital at Home programme (NOSPI). Our work included facilitating the selection process for the 10 Reference Centers, developing the full healthcare delivery and organizational model, and defining the system adaptations required to support the new service. We also assessed the long-term sustainability of the model within the Greek healthcare context.

Full patient and operational pathway mapping using BPMN methodology.
Assessment of operational conditions and identification of key service prerequisites.

Long-term sustainability assessment, reporting, and documentation aligned with ODIPY’s quality assurance framework.
